These terms and conditions outline the rules and regulations for the use of MEDICALLY FIT’s Website, located at MEDICALLYFIT.CA.

Personal Health Information Sharing Consent Form

To ensure the best client care, Medically Fit may share your personal health information with other health professionals only when and if clinically necessary. This could include:

  • The Polyclinic Family and Specialty Medicine Facility, where Dr. Housefather has his family practice and where he conducts his medical assessments and requisitions medical tests, and / or
  • Housefather Medicine Professional Corporation, and / or
  • Your family general practitioner (GP).

Limited information (such as your name, address and email address) may be used by Medically Fit to request your participation in a survey, to provide you with news about our programs, services and special events or for fundraising purposes to improve our health care services.

I hereby consent to the sharing of my personal health information from Medically Fit to the listed and / or involved health professionals and practitioners in my circle of care. I understand the ‘written’ communication may involve transferring information by electronic means utilizing secure servers. I also understand that I may withdraw my consent by contacting the Medically Fit Clinic by phone at (416) 994-7710.

Code of Conduct and Gym Etiquette

The purpose of the “Code of Conduct” is to ensure users of the facility at the Medically Fit clinic within the Champagne Centre understand the standards we have established for both staff and clientele who utilize the facility, to ensure a courteous, respectful and efficient facility.Violation of the code of conduct may lead to verbal warnings from staff, and a written warning if the situation requires.

Staff members have the right to remind you of a regulation (if it becomes necessary) to inform you of the standards in place at Medically Fit. We will ask for your cooperation in maintaining the code of conduct.

If we see that, despite communication regarding rule violation, or in cases of abuse or a serious offense, you will be temporarily or permanently expelled from the Medically Fit facility and may have your membership contract terminated. You will not be eligible for a refund of any products or services that have been pre-paid if termination is following breach to the code of conduct.

You must behave and speak respectfully to other members or users of the facility and staff. It is the policy of the Medically Fit Clinic that its employees, contractors, members and visitors of the facility have the right to work and utilize the facility in a violence free environment. Violence is the intentional use of physical force or power, threatened or actual, against oneself, another person, or against a group of community, which either results in or has a high likelihood of resulting in injury, death, psychological harm, maldevelopment, or deprivation (World Health Organization). Acts of violence may be verbal, physical, sexual, or psychological.

The use of profanity within the Medically Fit facility will not be tolerated.

Gym Etiquette

On arrival you must check in at the administration desk.

Performing physical activity in the Medically Fit gym requires appropriate clothing and shoes. You will remove heavily soiled shoes prior to entering the Medically Fit clinic, other moderately soiled shoes will be changed as quickly as possible with minimal walking around the facility. No one may use the gym in open toed shoes, barefoot, or shirtless.

Your body must be clean prior to utilizing the gym area, and limit the quantity of perfume, cologne, or other perfumed products that you wear during your workout. Exercise attire should be laundered after each workout.

Using the change room located in the gym is an appropriate space to change. You will not leave personal possessions in this space. Your personal possessions will be placed in the lockers provided. Medically Fit staff hold no responsibility for lost or stolen objects. Members must empty the locker at the end of their workout, or the padlock will be cut at closing time.

You will not take photos, or videos of other members or staff in the Medically Fit facility at any time without another person's express consent and awareness of the photo or video. Taking photos or videos of other members or staff may lead to immediate termination of your membership.

The use of more than one piece of equipment at a time is allowed. You must share the equipment with others during recovery periods of your workout. Throwing or dropping weights or dumbbells onto the floor is forbidden. All equipment used must be put away after use, leaving equipment out will not be tolerated.

You must bring and use a personal sweat towel throughout your gym session. You must clean all equipment after use by using either spray and wipe or wet wipes available. You must wipe over any areas in which sweat has been left, your head/ face was placed, and your hands have touched.

You must not adjust the temperature control of the gym, if the temperature is a concern, you may speak to a staff member and discuss the potential of adjusting the thermostat. Staff members reserve the right to remind you of any of these rules if necessary.

Gym Bookings

You will complete gym bookings online through the Medically Fit website (www.medicallyfit.ca). If you arrive without a booking and the gym is at capacity, we have the right to refuse entry.

Cancellation within 24-hours of your planned booking is non-refundable. Not showing up to a planned gym booking is non-refundable and will be noted, this may impact your ability to book appointments in future.

A maximum of three independent gym users may use the space at one time. 

If you are late to your planned open-gym session, you may not be able to stay beyond the planned gym-booking end time. (EG. If you have a 60-minute gym booking for 16:00, however, arrive at 16:20, you may still be required to end your gym session at 17:00.

A copy of this Code of Conduct and Gym Etiquette will be posted in a visible location within Medically Fit facility.

Photography, Media, and Internet Waiver

We may take photos and videos in and around the gym, staff and members to utilize for marketing purposes at any time. You waive any right to inspect or approve the finished photographs or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and you waive any right to royalties or other compensation arising from or related to the use of the image. You grant permission to us to use your image (photographs and/or video) for use in media publications including:

  • Videos
  • Email Blasts
  • Recruiting brochures
  • Newsletters
  • Magazines
  • General Publications
  • Websites and/ or Affiliates
  • Social Media
  • LinkedIn
  • Facebook
  • Instagram
  • Twitter
  • YouTube

WiFi

We provide the wireless network and its use is at our discretion. Your access to the network may be blocked, suspended, or terminated at any time for any reason. You agree not to use the wireless network for any purpose that is unlawful or otherwise prohibited and you are fully responsible for your use. The wireless network is provided "as is" without warranties of any kind, either expressed or implied. The password for the WiFi will be provided within the Medically Fit facility.

Cookies

We employ the use of cookies. By accessing WWW.MEDICALLYFIT.CA, you agreed to use cookies in agreement with the MEDICALLY FIT’s Privacy Policy.

Most interactive websites use cookies to let us retrieve the user’s details for each visit. Cookies are used by our website to enable the functionality of certain areas to make it easier for people visiting our website. Some of our affiliate/advertising partners may also use cookies.
